Student Drivers

All paperwork must be given to Joy Nguyen, SEP driver coordinator. Paperwork can be dropped off to the filebox outside office E318 in building 53 if the office is closed.

A student who volunteers to drive for a class field trip must fill out the paperwork below. For your convenience, we will process your paperwork for you if you adhere to our processing dates. If you miss our processing dates you will be responsible for getting all signatures and making sure Facilites and SEP have copies of the appropriate documents. You will receive an approval by email when you are an authorized driver, and you cannot drive until you receive this authorization.

Student Drivers
New drivers:
  1. Online Defensive Driving class application . If you need to drive before actually taking the class, Facilities can issue a one-time Temporary Defensive Driver card so that students can drive SEP vans for class fieldtrips before taking a Defensive Driver class. Just check the "yes" box and sign under the OTHER section at the end of the form. Once you complete the online defensive driver course, please print out the certificate, sign and date, and submit it to the van coordinator, Joy Nguyen.  Joy will submit your signed certificate to Dan Fernandez for signature and forward a copy to Susie Hernandez at Facilities.
  2. INF 1101 form --fill out top portion only, up to your signature and date
  3. Volunteer Driver Form -new form must be submitted every year (must be signed by Dan Fernandez).  Do not submit this form to Facilities.
  4. Copy of current driver license
  5. Copy of DMV driving record (has to be less than 30 days old). This can be obtained online http://dmv.ca.gov/online/dr/welcome.htm or at a local DMV office. It costs $2.00 for an online printout and $5.00 for a printout at a local DMV office.
  6. Driver Certification Form
Drivers that already have a CA State approved Defensive Driver Certificate:
  1. Volunteer Driver Form -new form must be submitted every year (must be signed by Dan Fernandez).  Do not submit this form to Facilities.
  2. Copy of current driver license
  3. Copy of your current Defensive Driving Certificate
  4. Driver Certification Form
CSUMB drivers returning to a new semester:
  1. Driver Certification Form
  2. Volunteer Driver Form -new form must be submitted every year (must be signed by Dan Fernandez).  Do not submit this form to Facilities.
Keeping Your Defensive Driving Certificate Current
Defensive Driving certificates are valid for 4 years, and the volunteer forms are valid for 1 year (expires at the end of a fiscal year ((June 30). In order to keep your defensive driving certificate current you need to update:
