| All paperwork needs to be sent to Joy Nguyen (Blg 53/E324) 582-4127 .
For your convenience, we will process your paperwork for you if you adhere to our processing dates. If you miss our processing dates you will be responsible for getting all signatures and making sure Facilites and SEP have copies of the appropriate documents. You will receive an approval by email when you are an authorized driver, and you cannot drive until you receive this authorization. |

New Drivers |
| All drivers must complete the paperwork below. If you need to drive before taking a Defensive Driver (DD) class, Facilities can issue you a one-time temporary card (check the "yes" box and sign under the OTHER section at the end of the online class form). Once you complete the online defensive driver course, please print out the certificate, sign and date, and submit it to the van coordinator, Joy Nguyen. Joy will submit your signed certificate to Dan Fernandez for signature and forward a copy to Susie Hernandez at Facilities. |

Extension |
| If your DD card expired and you need to drive before renewing your DD card, you can apply for an extension. Facilities will issue you a temporary DD card that is good until your next DD class. |

Privately owned vehicle on CSUMB business - required for reimbursement of driving expenses |
| If you want to drive your privately owned vehicle for CSUMB business, the following additional documents are required. (Mileage reimbursement paperwork is processed through the Finance Department.) |
- Current proof of California automobile insurance
- STD 261 form – good for a year

| Corporation employees |
| In addition to the required documents above, you need to fill out a Volunteer Driver Form if you are a Corporation employee. This form needs to be signed by Human Resources. A volunteer form is good up to a year. |

| Keeping Your Defensive Driving Certificate Current |
| In order to keep your defensive driving certificate current you need to update: |
- Driver License: When your driver license expires and you get a new one, give a copy to Joy Nguyen (Blg 53/E324) 582-4127.
- Defensive driving certificate: it’s good for 4 years. When it’s about to expire, sign up for a defensive driving training class. The Online Defensive Driving class application needs to be submitted to Facilities prior to taking the class
- Driver Certification Form - needs to be updated annually
- Volunteer Driver Form: (for student volunteers and Corporation employees only): good up to a year; expires at the end of a fiscal year (6/30) each year.)
- Proof of auto insurance: (only for those who wish to drive their privately owned vehicle for CSUMB business): has to be current and on file with Facilities
- STD 261 form: only for those who wish to drive their privately owned vehicle for CSUMB business): good for a year. This form has to be signed by the SEP department chair.
